Hospital to Home (H2H) is a newborn follow-up programme developed by Adara Development to help small and sick babies survive and thrive in the hospital and after they are discharged home.
The H2H programme has two components to maximise infant’s chances of a healthy life: Firstly, by supporting the
infant and family and strengthening hospital discharge processes in the newborn care unit, and secondly, by providing
a follow-up programme to support high-risk infants and their families after discharge.
In the first part of the programme, the ‘hospital’ component, H2H promotes care that encourages growth and
good brain development. The family-centred programme also focuses on parent education, including lactation,
breastfeeding, newborn development and essential newborn care. Parents are equipped with the knowledge and skills
required to care for their infant upon returning home.
