These counselling cards have been created as a resource for Community Health Workers to take to on their home visits as part of the Hospital to Home follow up visits. The counselling cards are used to guide them through the steps for each visit, and ensure each topic is covered . The images on the cards provide visual representation of the concepts to enhance learning.
