Hospital to Home (H2H) is a newborn follow-up programme developed by Adara Development to help small and sick babies survive and thrive in the hospital and after they are discharged home.
This resource is used to support facilitators to deliver the Hospital to Home Community Health Worker Training. This training is delivered by skilled healthcare workers, including the Community Midwife.
If resources are available, there are videos that can be used to enhance the training. It is also strengthened with an exposure visit for the Chief CHWs to the newborn care unit, and opportunities to interact and build relationships with the Discharge Coordinators.
