Hospital to Home (H2H) is a newborn follow-up programme developed by Adara Development to help small and sick babies survive and thrive in the hospital and after they are discharged home.
The CHW Manual provides basic information on topics related to care of the newborn at home following discharge from the newborn care unit. It also contains the exercises that CHWs will do during the sessions.
After the training, the CHW can use the manual as reference material during the course of their work. However, they are not
expected to take the manual with them when making home visits.
