Hospital to Home (H2H) is a newborn follow-up programme developed by Adara Development to help small and sick babies survive and thrive in the hospital and after they are discharged home.
Adara’s Hospital to Home (H2H) package encourages other facilities to implement the programme in other hospitals in low resource settings.
These implementation tools provide an outline job descriptions, documentation and evaluation metrics to measure success.
